Terms of Use
These are the terms for using this website and for buying care from Reclaim Power Telehealth. I have written them in plain language on purpose. If anything here is unclear, email me and I will explain it.
Reclaim Power Telehealth (“we”, “I”) is a telehealth practice run by Trang Ca Laube, a board-certified Family Nurse Practitioner and a Functional Medicine Academy Certified Practitioner (FMACP). By using this website you agree to these Terms.
The content on this website, including the articles, is educational. It is not medical advice, diagnosis or treatment, and reading it does not make you my patient. A provider–patient relationship begins only after you have booked or purchased care, completed the intake and consent forms in my health record system, and we have met for a visit.
Always seek the advice of a qualified clinician about your own situation.
I am licensed in Indiana, Colorado, Arizona, Washington and Utah. Any live visit with me, including the Lab Second Look, the Root Cause Blueprint review, the Results Walkthrough, membership visits and the Transformation Intensive, requires you to be physically located in one of those five states at the time of the visit. You confirm your location when you book.
If at the start of a visit you are located somewhere else, I cannot legally proceed. The visit will be rescheduled once at no charge. If it happens a second time, the visit is treated as a late cancellation under section 3.
Self-serve lab panels and the supplement dispensary run through Fullscript and are available in most US states, because no visit with me is involved.
Telehealth is not emergency care. If you are having a medical emergency, call 911 or go to the nearest emergency room.
Every price is published on the website. Prices can change, but the price you paid is the price you paid.
Paid appointments (the Lab Second Look and the Results Walkthrough). You may reschedule free of charge with more than 24 hours’ notice, using the link in your confirmation email. With less than 24 hours’ notice, or if you do not attend, I offer one courtesy reschedule; after that, the fee is forfeited. Once a visit has taken place, the fee is not refundable. If I have to cancel a visit, you choose between a full refund and a new time.
The Root Cause Blueprint. You may cancel for a full refund within 7 days of purchase, provided no lab order has been placed. Once a lab order has been placed, the cost of the labs is deducted from any refund, because the laboratory does not refund me. Once your results review has taken place, the Blueprint is not refundable; the written plan is yours to keep either way.
The Transformation Intensive. You may cancel for a full refund within 7 days of purchase and before the first visit. After the first visit, refunds are pro-rated for visits not yet held, less the cost of any advanced panel already ordered. The three-payment option is a payment plan, not a subscription: all three payments are owed for services used, and the plan stops on its own after the third payment.
Reclaim Membership. Month to month, with no contract. Cancel any time from your account or by emailing me; cancellation takes effect at the end of the current billing month, and that month is not refunded. Unused monthly visits do not roll over.
Credits. The $179 Lab Second Look fee credits toward a Root Cause Blueprint purchased within 30 days of the visit. The $99 Results Walkthrough fee credits toward a Blueprint if you continue. Credits are applied by me at the time of purchase; email me and I will send the adjusted link.
Refunds go back to the original payment method within 10 business days. To cancel or ask about a refund, email [email protected]. There is no phone line for this; email is faster and leaves us both a record.
Self-serve lab panels and supplements are sold by Fullscript and its partner laboratories, not by Reclaim Power Telehealth. When you buy them, Fullscript’s own terms, shipping, returns and refund policies apply, and questions about an order go to Fullscript. I do not manufacture, ship or guarantee any product, and I cannot refund a Fullscript purchase.
A lab panel you order yourself comes with Fullscript’s automated analysis and written plan. It is not a visit with me unless you add the Results Walkthrough.
Everything on this website, including the text, images, logo and articles, belongs to Reclaim Power Telehealth and is protected by copyright. You are welcome to read it, share links to it, and print a copy for your own use. You may not reproduce, sell or republish it without written permission.
This website links to other services, including Fullscript, Practice Better and Google Meet. I do not control those websites and am not responsible for their content or their practices.
Your use of this website is also governed by the Privacy Policy, which explains what information I collect and how it is protected.
To the fullest extent permitted by law, Reclaim Power Telehealth and its owner shall not be liable for indirect, incidental or consequential damages arising from your use of this website, reliance on its educational content, or third-party products and links. Nothing in these Terms limits liability that cannot be limited under applicable law, including for clinical care actually provided.
You agree to hold Reclaim Power Telehealth harmless from claims arising from your misuse of this website, your violation of these Terms, or your dealings with third-party services.
I may update these Terms. The effective date at the top will change when I do, and the current version is always the one on this page. The cancellation and refund terms that apply to a purchase are the ones that were posted on the day you bought.
These Terms are governed by the laws of the State of Indiana, without regard to its conflict-of-law rules.
