This explains what information Reclaim Power Telehealth collects, how it is used, where it is kept, and what you can ask me to do with it. Short version: I collect what I need to book and care for you, I keep health information inside HIPAA-compliant systems, and I do not sell any of it.
When you book, buy, message or subscribe, I collect what you give me: your name, email address, phone number, date of birth, the state you will be in at the time of a visit, and the answers on any intake or contact form.
When you become a patient, your health record system (Practice Better) also holds your health history, lab results, visit notes, written plans and messages between us. That is protected health information (PHI).
Like most websites, this one automatically records technical information such as your IP address, browser and device type, and which pages you visited.
Protected health information is handled in line with HIPAA and applicable state law. Specifically: your health record, intake, labs and clinical messages live in Practice Better; video visits run on Google Meet under a Google Workspace Business Associate Agreement; scheduling, payment and the emails and texts you receive from me run through GoHighLevel under its HIPAA agreement; self-serve lab panels and supplements run through Fullscript. Each of these providers has signed the agreements required to handle health information.
I do not put clinical detail in the website contact form, in text messages or in ordinary email. If you send it there, I will move it into your health record.
If you give me your mobile number, I may text you appointment confirmations and reminders, and occasional replies to something you asked. Message frequency is low; message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Your mobile number and your consent to receive texts are never shared with or sold to third parties for their marketing.
I do not sell or rent your information. I share it only with the service providers named above who help me run the practice and who are bound by agreements to protect it; when the law requires it; or to protect legal rights. If you ask me to send records to another clinician, I will, with your written authorization.
This website may use cookies and similar technologies for basic function and to understand traffic. You can disable cookies in your browser; the site will still work.
I use reasonable administrative, technical and physical safeguards, and I rely on the security of the HIPAA-compliant platforms named above. No system is perfectly secure, and I will tell you if a breach affects your information, as the law requires.
You may ask to see the personal information I hold about you, to correct it, or to have non-medical information deleted. Health records are kept for the period the law requires and can be sent to you or to another clinician on request.

This website and my services are for adults. I do not knowingly collect information from anyone under 18.
